Police Checks & Declarations
Reminder that Police Checks need to be completed every
3
years, and a
declaration
needs to be completed yearly for the 2 years between.
All Bench Staff are required to do Police Checks and yearly declarations. Bench Staff includes Coaches, Assistant Coaches, Trainers, Assistant Trainers, Managers and Parent Reps. If you are planning on being on the ice with Kids during practices you will also be required to fill out a police check or declaration.
Instructions for Police Checks: Vulnerable Sector Screening
If your bench staff member lives in an area patrolled by the OPP then they can use the link below to access the application form. If they live in another area they will have to go to the website for the police force that covers that area.
Applicants will be required to upload an BBMHA volunteer letter when applying for their record check. Please click on the following:

Choose “BEGIN RECORD CHECK” and follow the instructions.
Once the check is complete, an electronic record check is sent to the applicant's email address as a PDF attachment.
