Police Checks & Declarations (Blyth Brussels Minor Hockey)

All Bench Staff are required to do Police Checks and yearly declarations.  Bench Staff includes Coaches, Assistant Coaches, Trainers, Assistant Trainers, Managers and Parent Reps.
If you are planning on being on the ice with Kids during practices (even as a parent volunteer) you will also be required to fill out a police check or declaration. 

**New this year Vulnerable Sector Checks MUST be submitted to OHF**

 

When requesting a VSC from OPP

When you have your completed VSC Back:
Fill out and complete the declaration page
Submit the VSC and declaration to OHF 
You will need your Hockey Canada Registry Number (if you need help finding it please let us know)
